Transitional Care Unit


Our Transitional Care Unit offers:

  • 16 spacious private rooms with a private bathroom
  • Continuity of care with 24-hour nurse coverage
  • On-site physicians
  • Comprehensive on-site treatment—patients don’t have to leave the building
  • State-of-the art medical equipment and therapy gyms
  • Up to 3 hours of daily rehabilitation therapy
  • More intensive rehab than outpatient therapy services can provide
  • Focus on reducing risk of hospital readmission
  • Comprehensive patient and family education
  • Amenities such as a grooming salon, activities program, and separate dining room

Patients who may benefit from rehabilitation in the Transitional Care Unit can include those who are:

  • Recovering from hip or knee replacement surgery, fractures, other orthopedic conditions
  • Flare-up of chronic illness
  • Medically deconditioned due to surgery, injury or other complications.

Transitional Care Unit FAQ’s

  • Who covers the cost of a Transitional Care Unit? 
    Only Medicare and Medicare Advantage plans cover the cost of a stay at Transitional Care Unit.
  • Can I have visitors?*  
    Visitors are a key element in your recovery and we encourage visiting. *(Please be advised during COVID, there are no visitors allowed in the TCU for the safety of our patients and staff.)
  • What do I need to bring with me when I come to the TCU?
    Clothing so can you can dress in your own clothes and any other personal care items you require to make your stay comfortable.
  • How do I get into the TCU? 
    Your interdisciplinary team that is responsible for your care will identify if you are meet the criteria for the TCU and discuss your discharge options with you. If you are eligible, the nurse care manager will make a referral on your behalf to have you transferred when you are medically able.
