Staying at Montefiore Nyack Hospital


After your procedure has been scheduled, you will receive a call from the Admitting Department to pre-register. You may also contact the Admitting Department at 845-348-2383 or 845-348-2384 between the hours of 9:00 a.m. and 4:00 p.m. to start the pre-registration process. Either way, pre-registering will expedite the check-in process on the day of your appointment.

During the pre-registration call, you will be asked to provide insurance and demographic information. On the day of your appointment, you will be asked to sign the required statutory forms. You will also be asked to supply identification and your health insurance card(s). You will also be asked to supply government issued photo identification (acceptable forms listed below), health insurance card(s) and any referral forms or prescriptions issued by your physician or required by your insurance company.

Outpatient Registration

Patients who have not registered for outpatient services and diagnostic testing must first register at the Admitting Office located adjacent to the front lobby on the ground floor. Please be prepared to present your government issued photo identification (acceptable forms listed below), health insurance cards, and any referral, or claim forms when you register. You will be required to sign all appropriate forms at that time. You will then be directed to the appropriate patient care area.

Emergency Department Registration

Patients seen in The Emergency Department will need to have their government issued photo identification (acceptable forms listed below), and insurance cards. You may be asked to complete and sign the following documents:

  • Consent for Treatment
  • Authorization to Bill Insurance
  • Acknowledgement of Rights
  • HIPAA Joint Privacy Notice

Observation Service

Observation services are often provided in combination with outpatient emergency or surgical services. The observation unit affords you as a patient a place for further evaluation that may be necessary before a final determination is made about your care.

Patients in observation status enjoy many of the amenities offered to inpatients. These include placement in a hospital room, access to television, phone service, and in room dining. You may stay in the Hospital overnight while receiving Observation Services, but you should be aware that Observation is actually an Outpatient Service.

Your disposition as an observation patient is usually completed within 24 hours. In most cases a decision will be made within 24 hours whether to admit a patient to inpatient status or discharge them home to follow up in their doctor’s office.

Inpatient Registration

If your physician determines that you require inpatient level of care, he/she will make arrangements with the Admitting Department. A representative will then contact you to obtain demographic and insurance information.

When you arrive at the Hospital, you should first check in at the Admitting Office, located near the main entrance of the Hospital. Please be prepared to present your government issued photo identification (acceptable forms listed below), health insurance card(s), and any claim forms. You may also be asked to complete and sign the following documents:

  • Consent for Treatment

  • HIPAA Joint Privacy Notice

What to Bring with You

Please present government-issued photo identification:
Approved government issued identification includes:

  • U.S. Passport, Foreign Passport
  • State-Issued Driver’s License
  • State Issued Non-Driver’s Identification Card
  • Birth Certificate is acceptable if accompanied by another form of identification when no photo identification is available.
  • Patients under the age of 18 must have a copy of their birth certificate along with the parent/legal guardian’s photo ID.

You will also need your health insurance card(s), referral forms, order or prescription from your Physician and co-pays.

Authorization of Services

Health insurance is a contract between you and your insurance company. Many insurance companies require prior authorization before services are rendered. Be sure to contact your insurance company to obtain benefit and pre-certification information before your procedure. You should also verify that your physician has obtained the necessary authorization/pre-certification for your scheduled procedure. Without authorization, your insurance company may impose penalties or deny your claim. You may also be asked to reschedule your appointment.

Inpatient Dining

Patient dining is a program that offers choice. “At Your Request” provides bedside menus offering an extensive selection of food choices from which patients can call in their meal requests. Customer Service representatives assist patients by ensuring that selections adhere to any physician prescribed diet. The program allows patients to order, “What they want, when they want,” while the kitchen is still open. Meals are delivered to the patient’s room by a host or hostess within a set time.

One-on-one nutrition counseling services with a registered dietitian are available to help in developing a personalized meal plan and diet regimen to suit each individual’s lifestyle. Nutrition counseling can help guide people to make better food choices based upon various dietary restrictions or goals. An initial one-hour appointment includes a review of current food habits, education, and guidance on proper food choices based upon individual needs, counseling on various dietary supplements, vitamins and minerals, and meal planning tips for restaurants, and traveling. If needed, follow-up visits are available to fine-tune specific needs and address various nutrition goals.
