Montefiore Nyack Hospital is pleased to offer our patients quick and easy access to their medical records through MyChart. This tool provides important medical information, including:

• Medical History
• Procedures Performed
• Test Results
• Radiology Results
• Vital Signs
• Allergies
• Medications

Registered users can log in with their existing User ID. New users can quickly register on the site.

Release of Medical Records Requests

To request a complete copy of your medical record or radiology reports and images, or to send a copy to a third party, please download and complete the Authorization for Release of Health Information form. You may return the form to the Medical Records Department by:

• In-person requests: Given the heightened concerns related to the spread of COVID-19, we can provide copies of your medical records via secure email and/or mail a CD to you. We ask that you please fax/and or email your completed Authorization Forms. Bring the completed form to the Medical Records Department Monday – Friday between the hours of 8:00 a.m. – 4:15 p.m.
• Attorney requests can be mailed to the Medical Records Dept., Montefiore Nyack Hospital, 160 North Midland Avenue, Nyack, NY 10960.

There is a fee of .75 cents per page for copies of medical records. For additional information about the release of medical records, call 845-348-2527.

Birth Certificate Inquiries

Birth certificate inquiries for infants 3 months of age or younger, call 845-348-2885.

All other requests for birth certificates can be made by contacting the Village of Nyack.
